    • Patient Registration

    The first step in your care is registering for service. There are several ways to register, depending on what kind of service you need.

    How to Register for Outpatient or Rehabilitation Services

    If you are coming to the hospital for an outpatient procedure or test, or for rehabilitation services for the first time, please enter the hospital through the main entrance.

    A greeter or Patient Access specialist will direct you to a Patient Access desk to complete the registration process. Please make sure you have:

    • Your photo ID
    • Your insurance card(s)  

    Any estimated co-pay, deductible and/or co-insurance fees (where applicable) will be requested at this time.

    How to Register for an MRI or Surgery

    If you are coming to the hospital for an MRI, a surgical procedure, or your surgical pre-registration appointment, please enter through the main entrance and go directly to the Information Desk. Please make sure you bring:

    • Your photo ID
    • Your insurance card(s)

    How to Register After Business Hours

    If you arrive at the hospital after 7 pm and before 6 am (weekdays) or 8 am (weekends), please go to the Emergency Department to register.

    How to Register at the Emergency Department 

    If you arrive at the hospital for emergency services or to register for services after business hours, go the Emergency Department entrance and speak directly with the Emergency Department triage nurse at the window. If a nurse is not at the triage window, go through the solid wood door into the Emergency Department to register. After registering, emergency patients and family members should be seated in the Emergency Department waiting area near the triage window.

    The Emergency Department is always open and is staffed by a board certified Emergency Medicine specialist at all times.  

    How to Protect Your Private Health and Financial Information 

    Otsego Memorial Hospital uses the latest technology to help protect you from medical and financial identity theft. You can easily enroll in this identity protection program by presenting a photo ID and scanning your index finger during registration. The unique points on your finger will securely link you to your medical record. This extra layer of security helps prevent medical errors due to misidentification and speeds registration time at future visits. Your fingerprint is not stored and is not available to any authorities.
