Otsego Memorial Hospital Foundation Annual Healthcare Scholarships Applicaion (PDF)
Otsego Memorial Hospital Foundation Scholarships are available to individuals pursuing a career in a healthcare related field. The OMH Foundation will award scholarships totaling between $3,000 and $5,000.
The scholarships are open to students of all ages who reside in the Hospital's service area (Atlanta, Elmira, Frederic, Gaylord, Lewiston, Vanderbilt, Waters and Wolverine). Special emphasis will be given to students who are pursuing a degree in Nursing, Pharmacy, Respiratory Therapy, Physical, Occupational or Speech Therapy. ALL students pursuing a healthcare related degree are welcome to apply.
To qualify for the Foundation scholarship, an individual must meet the scholarship criteria and have proof of acceptance in a healthcare field of study at an accredited college or university. Please see application for the full listing of requirements.
