The OMH Family Practice office is located in Suite 210 inside the OMH Professional Medical Building at 829 N. Center Avenue in Gaylord, Michigan. The OMH Professional Medical Building is linked via an internal walkway to the main Hospital facility.
Our hours are Monday through Friday, 8 a.m. to 5 p.m.

OMH Family Practice has been designated a Blue Cross Blue Shield of Michigan Patient Centered Medical Home for 2010. A Patient-Centered Medical Home (PCMH) is a trusting partnership between a doctor led health care team and an informed patient. OMH Family Practice is home to the Move Smart, Eat Right Program -- developed for children and adolescents ages 2-18, this program focus on children whose weight puts them at risk for premature development of obesity related diseases. OMH Family Practice is an active participant in the Reach Out & Read Program. ROR is a primary care based literacy program offered through the Otsego Memorial Hospital (OMH) Medical Group.
